We believe improving employee health is essential for the success of your business. That's why we've built the Asuris Motivate rewards program into our metallic products. The program rewards eligible employees for completing a few simple wellness activities and rewards you with a discount.
When your employees complete an Asuris Motivate health assessment and a biometric screening within the first three months following your plan's effective or renewal date, they'll earn a $100 electronic gift card.
- Employees register on this site, navigate to Asuris Motivate, and complete the online health assessment.
- They can get a biometric screening one of three ways:
- Visit in-network doctors and have them fax results using our Physician Fax Form
- Visit a participating Patient Service Center
- Complete a home test kit and submit the test by mail
- Employees self-claim the electronic gift card code through Asuris Motivate.
When an employee participates and complete their wellness activities within the first three months after the plan's effective or renewal date, the group will be rewarded with a 3% discount on the next applicable month's premium* in aggregate, back to the plan's effective date.
Starting in 2024, employees’ spouses or partners** will be able to earn the same rewards as employees. Encourage your team to share the news about earning rewards.
Participating employees and spouses or partners can earn up to $50 more rewards when they complete their annual wellness exam, dental visit, vision exam and or health literacy programs at any point during the plan year. They’ll track and report these activities through Asuris Motivate.
