Members can use the Member reimbursement form to submit claims for medical, pharmacy, dental and vision services covered by Asuris. Members should only use this form if their health care provider is not submitting the claim.

The Patient review feature gives our members the ability to provide feedback on their experiences with the health care system. You can respond to your patient's comments about their experience with you by completing and submitting the Provider Response Form (PDF).
