Network termination or closing a practice

Refer to your provider agreement for instructions when submitting a network termination notice as language may vary. Always send network termination notices by certified mail or as indicated in your agreement. Include your name or practice name, tax ID, NPI number and the reason for network termination.

Claims appeals, balance billing or pricing disputes

The process for submitting an appeal depends on your issue. Review our pricing disputes and appeals information to ensure you are following the correct process.

Member claims submission

Members can use the Member reimbursement form to submit claims for medical, pharmacy, dental and vision services covered by Asuris. Members should only use this form if their health care provider is not submitting the claim.

Receiving secure emails from Asuris

Emails from us that contain personal health information are sent securely through Securemailbox Communications. Please view our secure email (Securemailbox) troubleshooting guide (PDF) to create an account in order to receive secure emails and, if needed, reset your password.

Respond to patient reviews

The Patient review feature gives our members the ability to provide feedback on their experiences with the health care system. You can respond to your patient's comments about their experience with you by completing and submitting the Provider Response Form (PDF).
